Help with added date, user name and dropdown list

I use the above programme DbArchive Scanning, but the date and user name doesn't come up when I select Validate Manual. Also I would like to add a dropdown list, please advise?

The problem with the DbArchive Scanning is that it doesn't appear anywhere on the Internet. This makes it very difficult to obtain additional information about the software and its functionality. However, if you have it installed, access its installation directory and look for user guide files.

They are typically found in form of a CHM, TXT, PDF, and HLP file format.
