There is no special procedure to get back your Premium Disconnect subscription. Simply re-download the application again and install it. Log in with the account you've created and the premium subscription will be activated. The software will display the premium membership information after you log in.
When installing, make sure to close your browsers for the setup package to configure everything without errors.
