From the BIR Relief program directory, go to Data folder and delete the ff. files:
uspalog.cdx
uspalog.dbf
That will reset the username and password of the program.

What is the default password? I cannot set my own password in the utility because the system is asking for the old password.
Try to visit http://www.recoverlostpassword.com and maybe you can find out a solution for your question.

... maintenance operations including password changing. From that menu click on <b>Change</b> <b>password</b> button, select ... the username in question and type your new password ...
