CCC001 toolbar is a generic toolbar created with Conduit Engine. The toolbar featured ways to access social services among custom items created by a specific user. If the toolbar entry is nowhere to be found, it means that the toolbar was installed through Add-ons/Extensions menu. These can be found in the Tools menu of your browser. You can remove the toolbar from that location.
