I tried to download FormMaker PRO Prescribe, but it appears there is no available setup package to obtain which also means that I can't test its functionality. There are some mentions on the Internet about it, but nothing conclusive regarding the supported file formats.
If you have the software installed, go to File > Open or File > Import and all the available files will be displayed in that dialog window.
