I've installed Instrument to MIDI and it appears it has a very detailed guide regarding the way you connect the devices to the PC. I recommend downloading the installation program, then simply tick the Show the manual option. Once the manual will open, click on the Using ipMidi.
As always, the software is available for download straight away. Make sure to use Run as Administrator when installing to properly add the virtual device drivers as well.
