If the software is available, then I believe it's not meant for the public use due to various security concerns. I've searched for the application, but I couldn't find anything about it. Not even Mastercard website contains information about Member Request Tool.
However, I recommend accessing its built-in Help section to look for contacts. This way you can get in touch with the official developers to ask for instructions.
