I have got the DVD with My Song Book but there is no username or serial number on it. I cannot use the package.

I recommend you call the company you bought the DVD from and tell them about your issue, they will know how to handle it. On the other hand you could still continue your tasks by downloading a similar software like SongBook it has a series of neat features like automatic formatting for current screen size, transpose songs very fast, define your own playlist for whatever reasons you like, etc.
