Unless you have manually changed the default save location, the application stores all of its files in the My Documents folder. You can find the data there, then either upload it to the cloud or copy it on a USB stick and move it to your new PC.
As far as transferring Golf Tracker for Excel goes, you can't really move an installed application from one PC to another unless they both have identical hardware configurations and use an identical Windows version. You can, however, download the tool on your new PC, install it and, in case you have the registered version, use the registration code on the new computer. Since GolfTRacker works on Excel, you should also make sure that you have Microsoft's spreadsheet application installed on the new PC.
