Unfortunately, Performance Appraisal System doesn't have a password recovery feature and the only to access the program is to uninstall and reinstall it.

If you use Windows XP or below you can go to Start > Control Panel > Add or Remove Programs. In the Currently installed programs box, click on Performance Appraisal System and then click Remove. If you have Windows Vista or above you need to go to Start > Control Panel > Programs > Programs and Features. Select the application and click on Uninstall.

If it doesn't work, try using Revo Uninstaller. Even if it can't uninstall the application normally, it will force it to uninstall by wiping out all traces of its presence from the system.

After that, try running AdwCleaner to make sure that there are no leftover registry keys that may contain the administrator information and reinstall the software.

Hi,

Although some time has passed since Dan Angel's answer, i'd like to point out that there is a way to recover the admin's password.
Search for, and open the file «worksystem.mdb» in access and go to the table Users.
Look for the user «admin» and the password is unencrypted in the field «password».

Regards
