W9 needed as soon as possible.

Hello, I am looking to obtain a W9 for 3D electric service. This web address was given to me to contact someone who could give me a W9. We are currently trying to send out our 1099's today and I do not have any tax ID information for 3D electric service. If someone could please send me an email with a W9 attached at [email protected] or give me a call at 1-262-439-7020 that would be much appreciated.
